Learn More About Diabetic Foot
Diabetes is one of the most challenging medical conditions worldwide in recent years. The number of patients has been increasing exponentially. Diabetic foot and ulceration are common complications among diabetic patients. The incidence of amputations of toes, fingers, and limbs is rising daily. Emergency treatment of diabetic foot ulcers and preventive foot care are crucial in preventing diabetic amputations.
Wounds and ulcers are common in diabetic individuals due to three important factors common in these patients:
- Increased Blood Sugar Levels: High glucose levels impair the body’s ability to heal wounds.
- Microangiopathy: Damage to small blood vessels reduces blood flow to the extremities.
- Neuropathy: Nerve damage leads to decreased sensation, making injuries go unnoticed.
These factors contribute to ulcer development due to decreased sensation, increased susceptibility to infection, and reduced blood circulation. Often, the initial event leading to ulcer formation is minor.
Preventing diabetic foot ulcers is vital, as mortality rates are high following amputations. Key preventive measures include:
- Strict Blood Sugar Control: Maintain optimal glucose levels to enhance healing and reduce complications.
- Foot Hygiene: Keep feet, web spaces, and nails clean and dry.
- Proper Footwear: Use soft, pressure-adjusted, properly fitting footwear both inside and outside the house.
- Skin Care: Apply moisturizers and oils to keep skin moist and supple, preventing dryness and itching.
- Avoid Cutting Calluses: Do not attempt to remove calluses yourself.
- Safe Nail Care: Cut nails after soaking them to soften them.
- Regular Monitoring: Consult a doctor immediately if any wound or ulcer develops.
Be alert to the following signs, which indicate the need for immediate medical attention:
- Persistent Ulcers: Wounds lasting more than four weeks.
- Infection Indicators: Foul-smelling discharge, exposed bone or tendon.
- Uncontrolled Blood Sugar: Elevated glucose levels and HbA1C.
- Swelling and Redness: Gross edema and cellulitis around the ulcer.
- Gangrene Signs: Wet gangrene and skin discoloration.
- Systemic Symptoms: Fever, increased local temperature.
- Circulation Issues: Absent peripheral pulses and severe pain.
Surgery is indicated in diabetic foot patients under certain conditions:
- Non-Healing Ulcers: Ulcers not responding to medical treatment after four weeks.
- Spreading Infection: Rapid infection and cellulitis around the ulcer.
- Deep Infections: Purulent discharge extending to joints and tendons.
- Exposure of Vital Structures: Bones and joints exposed in the ulcer.
- Septicemia: Systemic infection due to the ulcer.
Types of Surgical Procedures:
- Debridement: Surgical removal of dead or infected tissue.
- Skin Grafting: Covering the wound with a thin skin graft harvested from the thigh.
- Skin Flap Surgery: Using thicker skin with fat and sometimes muscle to cover the wound, especially when tendons or bones are exposed.
- Amputation: Removal of the infected area or limb when there is a risk of multi-organ failure or threat to life.
